K-9 Kutz with Jessie GROOMING RELEASE FORM
Your pet is very important to us!!! At K-9 Kutz with Jessie Dog Spa I would like to assure you that every effort will be made to make your pet's grooming experience as safe and enjoyable as possible. Please initial each paragraph and sign the bottom of the form. Thank you.
Health or Medical Problems
Occasionally, grooming can expose a hidden medical problem or aggravate a current one. This can occur during or after grooming. This also involves allergies and infected ears. Please let us know if your dog has sensitivities with soaps, shampoos, perfumes ect….. We use a hypo-allergenic shampoo unless specified to use one of our shampoo upgrades. We will not be held responsible in the event your dog has an allergic reaction. Please let us know if your dog has an ear infection, or you may suspect an ear infection. All medical expenses for veterinary care will be covered by the pet's owner.

Accidents
Although accidents are very rare, there is a risk when dealing with animals. Animals can make quick, sudden movements, grooming equipment is sharp, and although we use extreme caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quicking of nails, etc. Your pet’s safety and comfort is our number one priority. In the event an accident does occur, you will be notified of the accident. If we find the accident needs vet care, your dog will be taken to the nearest vet clinic. If accident/injury occurs due to matting, temperament/difficulty of dog, or self-mutilation, vet expenses will be the responsibility of the owner.
Current Vaccinations
K-9 Kutz with Jessie Dog Spa recommends that each dog is up to date on rabies and vaccinations. We also recommend having Bordatella, and for your dog to be on flea/tick preventative. K-9 Kutz with Jessie will not be held liable in the event that your dog gets sick, or parasites while in their care.
Fleas/Ticks
K-9 Kutz with Jessie strives to be a flea-free salon. If your pet is infested with fleas it will be sent home and you will be charged $25 for the appointment time. If we find just a few fleas on your dog then we will flea bath them in our natural flea shampoo and you will be charged an addition $5 to your groom. If you do not want them to receive one, you can reschedule your appointment after the problem is taken care of by you but there will be a $25 charge for the appointment time. Please check your dog over before booking your appointment . Please inform the salon when booking if you are aware that your dog has fleas or ticks! Ticks will be removed free of charge and you will be notified if any are found.
Muzzling and Aggressive/Dangerous Animals
K-9 Kutz with Jessie has the right to refuse any services at any time. In the event that your pet is too stressed or becomes dangerous to groom, K-9 Kutz with Jessie has the right to refuse grooming services, stop grooming services, or cancel grooming services at any time before, during, or after grooming and client will be charged a grooming fee (for what was done up until that point).
Muzzling does not harm your pet and protects both the pet and the groomer. In some cases, muzzling may even calm a stressed animal, allowing the grooming process to continue.
Matted Coats
Animals with severely matted coats require extra attention. Mats in an animal’s coat grow tight, and can ultimately damage and tear the animal's skin, which provides a breeding ground for parasite infestations. K-9 Kutz absolutely will not cause serious or undue stress to your pet by dematting. Mats can be very difficult to remove, and may require the pet to be shaved. Removing a heavily matted coat can cause nicks, cuts or abrasions due to skin growths trapped in the mats. Heavy matting can also trap moisture and urine near the pet's skin allowing mold, fungus or bacteria to grow, producing skin irritations that existed prior to the grooming process. After-effects of mat removal procedures can include itchiness, skin redness, self-inflicted irritations or abrasions and failure of the hair to regrow. In some cases, pets may also exhibit brief behavioral changes. Prevention is the best defense by scheduling regular grooming appointments, every 4-8 weeks. If your pet needs to be shaved to remove matting, by signing below, you acknowledge that you agree to this procedure, and any risk. There will be an additional charge for this process: it is time consuming, and causes extra wear and tear on grooming equipment.
Cancellations
Because we book on an hourly basis and cancellations can leave an empty block in the schedule that could have otherwise been used by another customer, we ask that any salon cancellations are made at least 24 business hours in advance.
Late pick up fees
We ask that you pick up your pet within a half hour of receiving our call that your pet is ready. Or we will give you a pick-up time that must be followed. There will be additional fee’s of $5/10 minutes late. We do not have room to hold your dog, and base our salon on a one dog at a time shop!
Missed, No-Show appointments
There will be a $25 fee for any appointment that is missed. Please understand we work by appointment only, and your missed appointment loses us some of our daily earnings, and wastes a spot another client could have had. The $25 will need to be paid before the next groom can be booked.
Photographs
This release form authorizes K-9 Kutz with Jessie and Ruffins Pet Store to take photos of your pet for a portfolio and online ads.
I have reviewed this Service Contract for accuracy and understand the contents of this contract. I affirm that I am the rightful legal owner of the dog(s) for which services are being rendered. I authorize this signed contract to be valid approval for future grooming services, permitting K-9 Kutz with Jessie and Ruffins Pet Store to accept telephone reservations for service without additional signed contracts or written authorization. I understand that pricing is subject to change. This agreement is valid for all pets that I currently own or will own in the future. I have read, signed, and agreed to the above.
